BACKGROUND: The effectiveness of bronchial thermoplasty (BT) has been reported in severe asthma, yet its impact on the different bronchial structures remains unknown.
OBJECTIVE: To examine the effect of BT on bronchial structures and to explore their association with clinical outcome in severe refractory asthmatics.
METHODS: Bronchial biopsies (n = 300) were collected from 15 severe uncontrolled asthmatics before and 3 months after BT. Immunostained sections were a
ssessed for airway smooth muscle (ASM) area, sub-epithelial basement membrane thickness, nerve fibers and epithelium neuroendocrine cells. Histopathological findings were correlated with clinical parameters.
RESULTS: BT significantly improved asthma control and quality of life at both 3 and 12 months and decreased the numbers of severeexacerbations and the dose of oral corticosteroids. At 3 months, this clinical benefit was accompanied by a reduction in ASM area (median values [25-75 IQR] before and after BT, respectively, 19.7% [15.9-22.4] and 5.3% [3.5-10.1], P < 0.001), in sub-epithelial basement membrane thickening (4.4 μm [4.0-4.7] and 3.9 μm [3.7-4.6], P = 0.02), in sub-mucosal nerves (1.0 ‰ immunoreactivity [0.7-1.3] and 0.3 ‰ immunoreactivity [0.1-0.5], P < 0.001), in ASM-associated nerves (452.6 immunoreactive pixels per mm2 [196.0-811.2] and 62.7 immunoreactive pixels per mm2 [0.0-230.3], P = 0.02) and in epithelium neuroendocrine cells (4.9 per mm2 [0-16.4] and 0.0 per mm2 [0-0], P = 0.02). Histopathological parameters were associated with asthma control test, number of exacerbations, and visits to emergency department (all P ≤ 0.02), 3 and 12 months after BT.
CONCLUSION: BT is a treatment option in severe therapy-refractory asthma that down-regulates selectively structural abnormalities involved in airway narrowing and bronchial reactivity, particularly ASM, neuroendocrine epithelial cells and bronchial nerve endings
INTRODUCCIÓN: se ha demostrado que la termoplastia bronquial (TB) es eficaz en asma grave, pero todavía se desconoce su impacto en las diferentes estructuras bronquiales.
OBJETIVO: examinar el efecto de TB sobre las estructuras bronquiales y explorar su relación con la expresión clínica en pacientes con asma grave refractaria.
MÉTODOS: se recogieron biopsias bronquiales (n = 300) de 15 pacientes con asma grave no controlada, antes y 3 meses después de TB. Se realizó inmunofijación de distintas secciones, valorando el área de músculo liso bronquial, fibras nerviosas y células neuroendocrinas del epitelio y el espesor de la membrana basal subepitelial. Los hallazgos histopatológicos se correlacionaron con los parámetros clínicos.
RESULTADOS: la TB mejoró significativamente el control del asma y la calidad de vida a los 3 y 12 meses y disminuyó el número de exacerbaciones graves y la dosis de glucocorticoides orales. A los 3 meses, este beneficio clínico fue acompañado por una reducción en músculo liso bronquial (valores medianos [25-75 IQR] antes y después de TB, respectivamente, 19,7 % [15,9-22,4] y 5,3 % [3,5-10,1], p < 0,001 ); en el engrosamiento de la membrana basal subepitelial (4,4 μm [4,0-4,7] y 3,9 μm [3,7-4,6], p = 0,02); en los nervios submusculares (inmunorreactividad 1,0 ‰ [0,7-1,3] e inmunorreactividad 0,3 ‰ [ p < 0,001); en los nervios asociados a músculo liso bronquial (452,6 píxeles inmunorreactivos/mm2 [196,0-811,2] y 62,7 píxeles inmunorreactivos/mm2 [0,0-230,3], p = 0,02) y en las células neuroendocrinas del epitelio (4,9/mm ^ {2} [0-16,4] y 0,0/ mm ^ {2} [0-0], p= 0,02). Los parámetros histopatológicos se asociaron con el ACT, el número de exacerbaciones y las visitas al Servicio de Urgencias (todos p ≤ 0,02), 3 y 12 meses después de la TB.
CONCLUSIÓN: la TB constituye una opción terapéutica en asma grave refractaria que disminuye de forma selectiva las anomalías estructurales involucradas en el estrechamiento de las vías respiratorias y la reactividad bronquial, en particular el área de músculo liso bronquial, las células epiteliales neuroendocrinas y las terminaciones nerviosas bronquiales.
