Beta-adrenergic receptor polymorphisms associated with length of ICU stay in pediatric status asthmaticus.

BACKGROUND:

During severe exacerbations, asthmatic children vary significantly in their response to high-dose continuous β(2) -adrenergic receptor (ADRβ(2) ) agonist therapy. Genetic polymorphisms have been identified within the ADRβ(2) that may be functionally relevant, but few studies have been performed in this population. Our hypothesis was that genotypic differences are associated with magnitude of response to ADRβ(2) agonist treatment during severe asthma exacerbations in children.

METHODS:

Children aged 2-18 years admitted to the ICU (intensive care unit) with a severe asthma exacerbation between 2006 and 2008 were eligible. Genotyping of the ADRβ(2) was performed.

RESULTS:

Eighty-nine children consented and were enrolled. Despite similar clinical asthma scores on admission, children with the Gly(16) Gly genotype at amino acid position 16 had significantly shorter ICU length of stay (LOS) and hospital LOS, compared to children with Arg(16) Arg and Arg(16) Gly genotypes. Children with either the Gln(27) Glu or Glu(27) Glu genotype at amino acid position 27 also had significantly shorter ICU LOS and hospital LOS compared to children with the Gln(27) Gln genotype. The Arg(16) Gly-Gln(27) Gln haplotype was associated with the longest ICU LOS, but this was not statistically different from other haplotypes.

CONCLUSIONS:

In this cohort of children with severe asthma exacerbations, ADRβ(2) polymorphisms were associated with responses to therapy. Knowledge of the genetic profile of children with asthma may allow for targeted therapy during acute exacerbations.

Polimorfismos del receptor beta-adrenérgico asociados con el tiempo de estancia en la Unidad de Cuidados Intensivos en niños con status asmático

antecedentes. Durante las exacerbaciones graves, los niños asmáticos varían significativamente en su respuesta al tratamiento con dosis elevadas continuas de agonistas de los receptores beta-2-adrenérgicos. Se han identificado polimorfismos genéticos dentro del agonista de los receptores beta-2-adrenérgicos que pueden ser funcionalmente relevantes, pero se han realizado pocos estudios en esta población. Nuestra hipótesis era que las diferencias genotípicas se asociaban con la magnitud de la respuesta al tratamiento con agonistas de los receptores beta-2-adrenérgicos durante una crisis asmática grave en niños.

METODOS. Fueron elegibles los niños con edad entre 2 y 18 años ingresados en la UCI por una exacerbación grave de asma entre 2006b y 2008. Se realizó un genotipado del agonista de los receptores beta-2-adrenérgicos.

RESULTADOS. Dieron su consentimiento y fueron incluidos 89 niños. A pesar de tener puntuaciones clínicas de asma similares al ingreso, los niños con genotipo Gly(16) Gly en el aminoácido de la posición 16 tuvieron una estancia en UCI y en el hospital significativamente más corta que los niños con genotipos Arg(16) Arg y Arg(16) Gly.

Los niños con genotipos Gln(27) Glu o Glu(27) Glu en el aminoácido en posición 27 también tuvieron una estancia en UCI y en el hospital más corta comparada con los niños con genotipo Gln(27) Gln. El haplotipo Arg(16) Gly-Gln(27) Gln se asoció con estancias en UCI más largas, pero no fue estadísticamente diferente de otros haplotipos.

CONCLUSIONes. En esta cohorte de niños con crisis graves de asma, los polimorfismos del agonista de los receptores beta-2-adrenérgicos se asociaron con respuestas al tratamiento. El conocimiento del perfil genético de los niños con asma podría ayudar a dirigir el tratamiento durante las exacerbaciones agudas de la misma.
